Job Summary: We're looking for a talented Case Manager RN to work remotely in Southern Michigan, with occasional travel to the Detroit area (up to 15% of the time). As a key member of reputed company, you'll be responsible for assessing, planning, implementing, and coordinating comprehensive case management activities to promote the overall wellness of our members. Your expertise will focus on evaluating medical needs, facilitating care planning, and collaborating with reputed company providers to ensure reputed company outcomes. Key Responsibilities:
- Conduct comprehensive assessments of members' needs and reputed company personalized care plans to address their unique requirements

Requirements:

- Reliable transportation and residency in one of the specified counties in Michigan
- Bachelor's degree in Nursing required
