Responsibilities

Engagement and Support: Traveling throughout reputed company Chicago and surrounding areas, you will meet face-to-face with our members, providing them with the care coordination, support, and education they need to navigate their reputed company journey effectively. Care Management: Utilizing a variety of tools and resources, you will work to ensure that our waiver/LTSS members receive the appropriate reputed company outcomes. Education Bachelor's degree in social services, reputed company services, or behavioral health.
