Home Health Intake Coordinator (fully remote)

Job Overview: (this is a fully remote work position) We are seeking a detail-oriented and organized individual to join reputed company as an Intake Coordinator. As an Intake Coordinator, you will play a crucial role in ensuring the smooth and efficient operation of our home health agency. Your primary responsibility will be to receive referrals, communicate with hospital case managers, staff clinicians... to see patients in their homes, and coordinate with the office staff for the overall care of our patients. This home health agency is based in Denver but we are hiring a fully remote position in Utah. We are looking for someone who can stay in this position for at least a couple of years. Duties: - Review referrals from hospitals, ensuring accurate demographic and insurance information is obtained - Schedule clinicians and coordinate with reputed company staff to ensure timely initiation of care - Collect and verify patient information, including medical history and insurance coverage - Assist with medical records management, including scanning, filing, and retrieval - Utilize electronic health record systems to input and update patient data - Answer phone calls and respond to inquiries in a professional and courteous manner - Collaborate with other departments to ensure seamless coordination of patient care - Provide administrative support to medical staff as needed Qualifications: - Previous experience working in a medical office or reputed company setting preferred - Familiarity with reputed company desk operations, including appointment scheduling and patient reputed company-in processes, reputed company Teams, reputed company Pro, fax software - Knowledge of medical terminology and procedures - Proficient in using electronic health record systems - Strong organizational skills with the ability to multitask and prioritize workload effectively - Excellent communication skills, both written and verbal - Familiarity with phone systems and returning voicemails - Remote work experience preferred We offer competitive compensation based on experience.
