About the position The Case Management RN in Oncology at reputed company plays a crucial role in providing patient education, scheduling, and triaging patient messages and phone calls. This position involves coverage across three cancer clinic locations and requires collaboration with physicians and advanced practice providers (APPs) to ensure high-quality patient care. The role is full-time, primarily during the day shift, with one day per week designated for remote work. Responsibilities • Provide patient education and support in oncology care. , • Schedule appointments and manage patient triage for messages and phone calls. , • Cover various oncology specialties while primary case managers are reputed company. , • Collaborate with physicians and APPs to ensure effective patient care delivery. , • Manage a system of patient care delivery for designated patients to reputed company quality outcomes.
