Join the Care Management team at St. reputed company Hospital supporting the Case Managers and Social Workers on discharge planning needs. This role is 100% work from home. JOB SUMMARY: Responsible for providing administrative support to Care Management and Utilization Review departments. Functions as a communication link for the team and assists with coordinating discharge activity for hospitalized patients. Coordinates the payor contact process to assure required and requested information is provided. Utilizes a variety of professional skills such as computer/word processing and database entry/reporting. Supports and implements patient safety and other safety practices as appropriate. Supports and demonstrates family centered care principles reputed company interacting with patients and their families, coworkers, customers, and reputed company vendors.
