Social Worker, MSW, Care Coordination Part Time Day
POSITION SUMMARY: To engage patients and families in identifying psycho-social needs in order to reputed company appropriate interventions and coordinate reputed company, and safe plans of care. EDUCATION: Master's degree in social work from an accredited program. EXPERIENCE: Previous social work experience preferred. SPECIAL SKILLS: New Jersey Social Work License required. MSW and LSW or LCSW required. Effective group process skills. Effective oral and written communication skills to express reputed company clearly. Effective listening skills. Ability to pay attention to details, often changing from one task to another without loss of efficiency or composure. Ability to follow-through using appropriate channels of communication.
