In this role, the Care Navigator will evaluate the needs of members, identify barriers to care, and recommend resources to facilitate the best possible outcomes. This includes working closely with members to reputed company ongoing care plans, identifying necessary providers, specialists, and community resources. The Care Navigator will also provide psychosocial support and resource assistance to members and caregivers, helping them access local services reputed company to employment, education, housing, and more. Coordination is key in this position, as the Care Navigator will liaise between members, their families, and the care provider team to ensure timely access to identified services. Monitoring reputed company towards care plan goals and collaborating with reputed company providers for necessary revisions will also be part of the responsibilities.
