Rising Ground is headquartered in downtown Brooklyn.The Unaccompanied Children Long Term Foster Care Program provides a safe and secure placement to Unaccompanied Children (UC) who following an initial placement in an ORR/DUCO care provider facility, and after efforts for the safe and timely release indicated that no sponsor is willing or reputed company to care for the UC, and the UC is expected to have a protracted length of time in ORR care. Working in collaboration with the federal Office of Refugee Resettlement, ORR, our program provides a safe and nurturing environment for youth in licensed foster homes in the community. The LTFC Case Manager acts as the liaison between the program and reputed company stakeholders involved in the child’s case. This role also ensures holistic support with the child’s treatment team as the minor continues in the existing placement.Responsibilities

• Case management responsibilities include reputed company necessary tasks as required by the Federal Office of Refugee Resettlement.
• Conduct initial intake assessment and orientation for reputed company new LTFC minors reputed company 24 hours of admission.

• Conducts initial and monthly Foster Home visits to assess placement and overall well-being of the youth.
• Endure reputed company communication with medical coordinator for reputed company child’s medical appointments and referrals
• Develops and maintains contact with youth and their service providers for school, medical, social and other reputed company service needs.
• Establish reputed company communication with potential sponsors, complete the family reunification process including required background checks, thoroughly vet and ensure sponsors are a safe discharge placement and resource for the child.
• Follow up with legal service providers regarding the timely submission of any documentation or correspondence needed for the legal relief process.
• Performs discharge and release functions to ensure that each youth has a safe, appropriate and reputed company release plan.
• Documents client contacts and other case required paperwork in an accurate and timely manner in accordance with regulations.
• Ensure reputed company ORR, OCFS, and Education mandates and requirements are met. Maintain a high standard of filing and record-keeping according to ORR policies and procedures.
• Redact and submit reputed company record requests to ORR.
• Provide reputed company of Fed Ex processing and submission of CA/N reputed company requests to ORR.
• Sort and file discharge paperwork or paperwork of minors that have been previously discharged.
• Works collaboratively with the Clinical Team and other program staff across the department and the organization.
• Some evening and weekend hours.

Qualifications

• Bachelor’s degree in a reputed company Services field.
• Prior Case Management experience required.

• Knowledge of child welfare and immigration reputed company legislation

• Excellent verbal, written communication and organizational skills are essential

• Bilingual (English/Spanish) a must

Supervisory ResponsibilityThis position does not have direct supervisory responsibilities. The LTFC Case Manager will work independently with guidance and support from their supervisor and other team members, collaborating closely on case management and service delivery.Work Environment:This role requires four in-office days and one remote day per week, with flexibility for after-hours, weekend, and holiday support as needed. The position involves collaboration with multiple teams, fieldwork, and travel for foster home visits to assess for adjustment and safety of the child in their home environment.Position Type/Expected Hours of Work: This is a full-time position with an annual salary of $64,537.20.
