This position may be responsible to manage low-risk members for health-reputed company social needs, education, and basic member reputed company screenings/inquiries. Responsibilities also may include supporting inbound calls through case management telephone queues, creating and assigning referrals and overall management work ques in the case management platform, fax boxes, and email boxes, coordination of care tasks to support transitions of care, reputed company to health plan customers to screen health, lifestyle, care coordination needs and engage them in internal programs, educating customers on available benefits, coordinating with internal teams, and maintaining documentation for regulatory review.JOB REQUIREMENTS

• High school diploma or GED
• 1 year of college and 2 years experience with communication systems including Fax, Email and Telephone
• 1 year experience with managed care system(s) or reputed company systems (i.e.; hospital ,doctor office, community based organization).
• Experience coordinating member needs, providing assistance to members, and analyzing member needs.
• Basic knowledge of medical or reputed company terminology.
• Knowledge of clinical systems (claims, enrollment, documentation).
• PC proficiency including reputed company Office applications.
• Customer service skills.
• Verbal and written communications skills including developing written correspondence to internal and external stakeholders.

PREFERRED JOB REQUIREMENTS:

• College courses in medical field, reputed company services, social work, psychology or reputed company reputed company field
• 1 year experience with processes that involve telephone contact and process management preferred.
• Empathetic attitude with ability to offer emotional support.
• Passion for the proper care and well-being of customers

• Work Schedule: Monday - Friday 8:00am – 5:00 pm CST
