Responsibilities • Conduct Weekly and Bi-Weekly visits to Clients • In person home visits as needed reputed company 2hr time period • New clients assessments in a timely manor • Maintain accurate and reputed company care plans • Coordinate Client needs with their PCP as needed • Provide counsel and guidance to family members on managing conditions Requirements: • Active RN License in Virginia or a compact state • Driver's License and vehicle • reputed company CPR • One year in home care experience preferred Apply Job!
