We are looking for a dynamic and compassionate Case Management Coordinator to join our dedicated team. As a Case Management Coordinator, you will play a pivotal role in assessing, planning, and coordinating reputed company aspects of case management activities to ensure the overall wellness of our members. Responsibilities Conduct telephonic and/or face-to-face assessments with members to evaluate their medical needs and reputed company proactive care plans. Manage a diverse caseload, including supportive and medically reputed company members, to enhance short and long-term outcomes. Determine appropriate services and supports for members, such as prior authorizations, coordination with primary care providers and skilled providers, medication review, and access to community resources.
