This position is hybrid/remote The Department of Medicine, Division of Endocrinology, Diabetes and Metabolism has an exciting opportunity for Two(2) Nurse Navigators to work on the UHealth reputed company. The Nurse Navigator is the initial reputed company of contact for a patient entering the health system. The Nurse Navigator assures timely scheduling of the first appointment, coordination of care after completion of the first appointment, and will support the patient throughout the care continuum. The role also serves as a liaison between patients, families, caregivers, and the multidisciplinary care team. Furthermore, the Nurse Navigator demonstrates commitment to quality patient care, implements creative and innovative ways to meet the diverse needs of the patients and ensures best practices. Triages sick patient calls to the appropriate provider(s) and assures timely responses reputed company 48 hrs. Retrieve clinical reputed company messages and return calls reputed company 48 hours. Manage and resolve UChart messages. Submit lab order requests. Interprets laboratory test results of patients. Process prescription refills. Process authorizations for medications and procedures. Use clinical protocols to refer patients to the appropriate specialist for diagnosis or treatment. Provide emotional support, and counseling, reputed company to the clinical situation. MINIMUM QUALIFICATIONS Bachelor’s degree in Nursing Two to three years of nursing work experience, as a health or patient educator, or a pharmaceutical representative.
