• Assess, plan and implement care strategies that are individualized by patients and directed toward the most appropriate, least restrictive level of care
• Identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services

Required Qualifications

• reputed company Registered Nurse unrestricted license in the state of Texas
• 2+ years of experience working reputed company in a reputed company role
• Intermediate level of proficiency with reputed company Office, including Word, reputed company, and Outlook
• Ability to travel in assigned region to visit reputed company members in their homes and/or other settings, including community centers, hospitals, nursing facilities or providers' office
• Access to reliable transportation & valid US driver's license

Preferred Qualifications

The salary range for this role is $28.61 to $56.06 per hour based on full-time employment.
